This applies to forms on your website and forms in the athlete app. The prompt looks the same in both.
What a new visitor sees
Someone whose email is not in your organization goes straight through:- They answer your form questions.
- On the Login/Signup page they create an account (name, email, password).
- They continue to any product or scheduler pages you attached, then submit.
What a returning visitor sees
Someone whose email already has an account with you sees a sign-in prompt appear as soon as they enter that email:- The prompt has their email pre-filled and asks for their password.
- A Forgot password? link lets them reset it without leaving the form.
- If they close the prompt, their form answers are kept, and a Member Login option stays available so they can sign in when ready.
Why it works this way
Form responses attach to a person’s profile in People. CoachIQ cannot attach a response to an account without confirming the person submitting owns that email, so it asks them to prove it by signing in. This is what keeps a returning parent’s new registration, payment, and bookings on the same profile as last year’s.What to tell a parent who asks
Copy and paste this if someone emails you about the prompt:That sign-in box means you already have an account with us from a previous registration. Enter the password you set up before, or click “Forgot password?” to reset it. Your answers on the form are saved while you sign in.

Only by removing the Login/Signup requirement from the form’s Connections. Without it, responses are collected but are not attached to a profile, and Contact Info fields will not sync to People. See Creating Forms.

Someone in the family probably did, often the other parent or the athlete. Search the email in People to confirm, then have them use Forgot password?.
